The NASA Mishap Information System (NMIS) is a custom-developed system for capturing mishaps, close calls and hazards, as required in NASA Procedural Requirement 8621.1C, NASA Procedural Requirements for Mishap and Close Call Reporting, Investigating, and Recordkeeping. The NMIS Quick Entry Mishap and Hazard Reporting mobile app provides a quick an easy way for all NASA civil servants and support service contractors to report to their center Safety office any safety related events, unsafe acts, or hazards they have observed.
